What the inspector wrote

At 9:20am on 2/26/2026, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Paolo Sarmiento to conduct a case management visit related to deficiencies discovered through the course of a complaint investigation. During today's visit, LPA observed a census of 6 school aged children supervised by 1 staff. During the course of a complaint investigation, LPA determined that the facility did not follow Title 22 requirements to report to licensing any unusual incident that threatens the physical or emotional health and safety of a child. Based on staff interview, the facility was told by a parent that they brought their child to a physician due to nail polish binding to their scalp after an incident at the facility. The facility did not report this to licensing. A Type B Title 22 deficiency is cited on the following LIC809-D Exit interview conducted and report was revealed with the facility representative, Paolo Sarmiento. A notice of site visit was given and must remain posted for 30 days.

This poses a potential health, safety, or personal rights risk to persons in care.

    101299(a) The licensee shall provide care and supervision as necessary to meet the children's needs.This requirement was not met as evidenced by: Based on interview, the licensee did not comply with the section cited above due to staff not providing supervision adequate to notice that nail polish was being used by children in care, leading to a child sustaining nail polish on their scalp. This poses an immediate health, safety, or personal rights risk to persons in care.
