Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During file review it was discovered S1 did not have a current Mandated Reporter certificate on file. The certificate on file expired on 2-21-2024.

Based on the observation, the Licensee did not meet the above regulation which poses an potential Health risk to the children in care. During facility tour the LPA observed two solid waste containers that did not have tight fitting lids.

Based on the record review, the Licensee did not meet the above regulation which poses an immediate Safety risk to the children in care. During review of the facility roster it was discovered S1 has a fingerprint clearance but it not associated to the facility or any facility under the same Licensee. During interview it was disclosed S1 has been employed at the facility since April 2024. $500 civil penalty assessed.
