What Happened
Annual inspection identified serious staffing compliance issue. Type A citation issued due to staff not having received required criminal record clearances. Opening/closing staff CPR certification expired or expiring.
Observations
- Staff criminal record and child abuse index clearances not completed (immediate risk - Type A citation)
- Opening and closing staff CPR/First Aid certification expires 4/2024

Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During file review it was discovered S1 did not have a current Mandated Reporter certificate on file. The certificate on file expired on 2-21-2024.

During review of the facility roster it was discovered S1 has a fingerprint clearance but it not associated to the facility or any facility under the same Licensee. During interview it was disclosed S1 has been employed at the facility since April 2024. $500 civil penalty assessed.
