Based on confidential interviews, and record review LPA is unable to corroborate because records revealed that an incident was provided to the parents of a child who did receive a scratch and interviews revealed C1 self harms self. Based off of limited information provided from the reporting party it is unclear what child was injured and whether a incident report was provided. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the allegations did or did not occur, therefore the allegations are unsubstantiated. An exit interview was conducted and a copy of this report along with the appeal rights were provided to Director Blanca Flores. A notice of site visit was handed to licensee and must remain posted for 30 days.
