What the inspector wrote

Based on conflicting statements, LPA is unable to corroborate the allegations Staff did not provide proper supervision. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the allegations did or did not occur, therefore the allegations are unsubstantiated. An exit interview was conducted and a copy of this report along with the appeal rights were provided to D Blanca Flores. A notice of site visit was handed to licensee and must remain posted for 30 days.

    101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation...This requirement was not met as evidenced by: Based on interviews C1 was instructed on how to clean themselves by S3 from the doorway of the restroom after having a potty incident, which resulted in C1 being in the restroom for approximately 30 to 40 minutes attempting to clean themselves with wet paper towels.
