Inspector’s narrative

What the inspector wrote

Licensing Program Analysts (LPAs) Taadhimeka Zeigler and Justin Giese arrived at the facility on date and time listed. A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 06/17/2021. It indicates that on 06/14/2021, child #1 ran out of the classroom into the breezeway, but still behind the gate, unsupervised. Child #1 was intercepted by staff in the front office. During today’s inspection, LPAs reviewed the files for staff #1 and child #1. LPAs also interviewed the Asst. Director, Daniela Nava. After the incident occurred, the facility placed staff #1 on administrative leave and conducted their own investigation resulting in the termination of staff #1. Based on the information gathered, the following violation has been identified: See LIC809D for cited deficiencies. 101229 (a)(1) Responsibility for Providing Care and Supervision The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to facility staff.
