Based on observation and interview, the licensee did not comply with the section cited above in the LPA observed the blacktop surface to be raised and cracked by the growth of large tree roots, which is a potential tripping hazard. In addition, although the artificial rubber turf installed was observed to have some padding underneath that provided a bounce, the material was hard as a rock and rough to the touch due to the wear and tear, which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision - The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time. Supervision shall include visual observation. This requirement has not been met as evidenced by: On 06/14/2021, C1 ran out of the classroom into the breezeway, but still behind the gate, unsupervised. Child was intercepted by staff in the front office. This poses an immediate risk to the health and safety of children in care.
