What Happened
Annual inspection identified multiple Type A and Type B violations. Facility has cleanliness issues with unclean floors and unsanitary bathrooms (leaking toilet). Hazardous storage violations present with cleaning chemicals and bleach stored in accessible cabinets. Missing safety equipment (no CO/smoke alarms in classrooms) and incomplete staff records. Lead testing records under review.
Observations
- Floors and rugs unclean and unvacuumed in children's classrooms (licensee cleaned before LPA departure)
- Toilets unclean with one leaking water
- Bleach jugs, wipes, toilet and floor cleaners, surface cleaners, and air fresheners stored in accessible upper and lower cabinets in children's classrooms
- No carbon monoxide or smoke alarms observed in classrooms
- Missing Mandated Reporter Training for 2 staff members
- Missing immunization records for 2 staff members and 2 children
- Missing LIC503 Health Screening for 2 staff members
- Missing proof of administrative units for director and Assistant Directors
- Missing LIC701 Physician's Reports for 3 children
- Lead test records incomplete (completed 11-15-2022, under review)
