Reporting Requirements: The items specified in (d)(1)(A) through (H) shall also be reported to the child’s authorized representative.This requirement was not met as evidenced by: Based on record review, and interviews, CCC was notified of a second confirmed case of Hand, Foot, and Mouth Disease; however, notification was not provided till next day. Furthermore, nine families were not informed of the outbreak and did not receive the notification letter. This failure to provide timely notification poses a potential risk to the health, safety, and personal rights of children in care.
