What the inspector wrote

On Friday, January 30, 2026 Licensing Program Analyst (LPA) Jesse Gardner conducted a Case Management visit to address additional information that was revealed during the course of the investigation into complaint #10-CC-20251211085210. On December 09, 2025, there was a physical altercation between Child 1 (C1), and Child 2 (C2). Internal record review with CCL confirmed the facility did not report the incident with the required time frame according to Title 22. An exit interview was held with Program Lead Monica Meras. A copy of this report was issued, along with copies of the LIC809-D, and Notice of Site visit, and Appeal Rights. This report shall be public record for three years.

    Responsibility for Providing Care and Supervision: (a) The licensee shall provide care and supervision as necessary to meet the children’s needs. This was not being met as evidenced by: Based on interviews conducted, staff were not able to provide adequate supervision to meet the needs of C1, and C2 while they engaged in a physical altercation. This is a potential personal rights risk to children in care.

This was not being met as evidenced by: Based on record review, and staff admission, the facility did not report the incident that occurred on 12/9/2025 to CCL by the regulation. This is a potential health and safety risk to children in care.
