What Happened
LPA conducted a case management inspection in response to a self-reported unusual incident. The incident involved an infant walking out through a doorway into the parking lot, and the child was returned safely by a parent about a minute later.
Observations
- An infant walked out through the doorway to the parking lot when a teacher opened the door.
- The door that was opened was not supposed to be opened, and staff said they had been trained on it before and after the incident.

A teacheropened the door to the parking lot and a child was able to exit the room without the teacher seeing the child exit.This poses an immediate risk to the Health and Safety of the children in care.Please note that a $500.00 civil penalty is also being assessed.
