What Happened

LPA conducted a case management inspection in response to a self-reported unusual incident. The incident involved an infant walking out through a doorway into the parking lot, and the child was returned safely by a parent about a minute later.

Observations

  • An infant walked out through the doorway to the parking lot when a teacher opened the door.
  • The door that was opened was not supposed to be opened, and staff said they had been trained on it before and after the incident.
