Case management inspection for lead testing exceedance. Water outlet A in infant room kitchen tested at 11.0 ppb (above safe levels). Facility remediated by replacing outlet on 3/17/2025 and providing bottled water. Type B deficiency cited.

LPA conducted a case management inspection in response to a self-reported unusual incident. The incident involved an infant walking out through a doorway into the parking lot, and the child was returned safely by a parent about a minute later.

A tele-inspection for a self-reported incident found that an infant was given another infant's bottle and drank some of the wrong formula. Staff later retrained on the infant feeding policy, and the bottles were reported to be labeled with the children's names and dates.
