What Happened
Case management inspection for lead testing exceedance. Water outlet A in infant room kitchen tested at 11.0 ppb (above safe levels). Facility remediated by replacing outlet on 3/17/2025 and providing bottled water. Type B deficiency cited.
Observations

- Outlet covered and posted with required signage for no-use
- Outlet replaced on 03/17/2025
