PERSONNEL REQUIREMENTS
Based on interview, the licensee did not comply with the section cited above. The licensee did not have a CPR/1st aid card for the LPA to review and by the Licensee's own admission, the CPR/1st aid training had not been renewed. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on child record review, the licensee did not comply with the section cited above . In reviewing children records/files, LPA did not observe LIC 627 Consent for Emergency Medical Treatment forms for Child #2 and Child #3. This poses/posed a potential health, safety or personal rights risk to persons in care.
