What the inspector wrote

Licensing Program Analyst (LPA) Elyse Jones arrived at the facility to conduct a Case Management inspection for the purpose of addressing separate matters that were discovered after a complaint investigation was concluded. After further review of pertinent documentation received for a complaint dated June 19, 2024, the Department has determined the unsubstantiated finding was an error. Documentation revealed there were statements from staff stating they observed the child with scratches upon pulling up his/her sleeves to wash their hands. Staff stated the child “could have” gotten the minor scratches while at the facility but the staff were unable to determine when or how the child got the scratches. The staff attempted to ask the child what happened but the child did not respond and he/she was not showing any signs of distress. The child did not receive outside medical attention, therefore, the Department did not receive any medical documentation. Additionally, it was noted the scratches were discovered upon pickup by the Authorized Representative, however, the scratches were not immediately reported. See LIC 809-D for deficiency cited A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Rob Kirschenmanm, Licensee .
