What the inspector wrote

On May 20, 2025, Licensing Program Analyst (LPA) Elyse Jones arrived at the facility to conduct a Case Management inspection in response to the receipt of an Unusual Incident Report (UIR) from the facility. The UIR was received by the licensing agency on May 8, 2025. It indicates at approximately 5:00PM during transition from the outdoor playground to the indoor playground two children were left in the hallway for about two minutes. When staff observed the children, the children were immediately taken to the Assistant Director without injury. The children’s Authorized Representative(s) were notified during pickup. During this inspection interviews with Pertinent Parties were conducted and documentation was collected. The facility states they have a "name to face policy, headcount system and a Star System put in place for transitions," however, it was determined there was an absence of supervision. Based on the information gathered the following violation has been identified: 101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, e xcept as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. See LIC809D for cited deficiencies LPA inform ed Rob Kirschenmanm, Licensee that this report dated 5-20-2025 documents one Type A citation which shall be posted for 30 consecutive days as there was an immediate risk to the Health, Safety, or Personal Rights of children in care. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224) was provided to facility during this inspection. The LIC 9224/Type A citation must be provided to parents/guardian of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for the verification. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed w ith Rob Kirschenmanm, Licensee .
