On 05/07/2025, Licensing Program Analysts (LPAs) Raymond Moorehead and Perla Ordones arrived at the facility for a separate matter. LPAs met with Assistant Director Jennifer McClintock, toured the facility, took census, and discussed the following. LPAs obtained information that an infant was being swaddled while in care during nap time. This has been determined to be a violation of Title 22 Regulation 101430(3)(C), Infant Care Activities, which states that "an infant shall not be swaddled while in care." See LIC809-D for cited deficiency. LPAs Raymond Moorehead and Perla Ordones informed Assistant Director Jennifer McClintock that this report dated 05/07/2025 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of the children in care. Also, LPAs Raymond Moorehead and Perla Ordones informed Assistant Director Jennifer McClintock to provide a copy of this licensing report dated 05/07/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgment of Receipt of Licensing Reports (LIC 9224), or other written statement, must be placed in the child’s file for verification. A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00. Exit interview was conducted and report was reviewed with Assistant Director Jennifer McClintock.
