What the inspector wrote

On Thursday, March 19, 2026 Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management visit in reference to information that was revealed during complaint number 10-CC-20260211151121. During the investigation into the complaint, it was revealed there was an incident that occurred on 12/16/2026 between Child One (C1), and Child Two (C2) where C2 slapped C1 causing C1’s nose to bleed. The facility did not report the incident to Community Care Licensing neither by phone nor Unusual Incident Report. Based on information gathered, a violation of Title 22 was identified and a citation was issued. An exit interview was held with Site Supervisor Lizeth Ouellette and a copy of this report was issued, along with copies of the LIC809-D, and a Notice of Site visit. This report shall be public record for three years.

This requirement was not being met as evidenced by: Based on interview and record review, the facility did not submit a UIR regarding the incident that occurred on 12/16/2026 involving C1 and C2 where C1's nose bled as a result of being slapped by C2. This is a potential health and safety risk to children in care.

    Admission Agreements:(d) Modifications to the original admission agreement shall be made whenever circumstances covered in the agreement change, and shall be dated and signed by the persons specified in (c) above.This requirement was not being met as evidenced by: Based on staff interview, and confidential interview, C1's parent was not given proper notice for the facility to reduce C1's hours. This is a potential personal rights risk to children in care.
