Inspector’s narrative

What the inspector wrote

bag of garbage on floor outside of daycare bathroom, broken cabinet under kitchen sink, holes in screened security door and a bag of trash on the floor in the kitchen. Licensee stated that the FCCH is cleaned twice daily and toys wiped down every weekend Licensee did remove the trash bags from the FCCH during inspection. Regarding the allegation that the facility is malodorous, it was alleged that the FCCH had a really strong odor like a recycling center. LPA Gerth noticed a strong smell of urine from the carpets in the daycare area and a smell of spoiled/rotten food. LPAs also observed a crate of string beans on the kitchen cabinet that were starting to mold. Licensee confirmed that licensee also smelled urinej and may be the bathroom area. Based on observation, the preponderance of evidence has been met and the allegations that the facility is unkempt and the facility is malodorous are substantiated. The facility is being cited three Type B violations of Title 22, Section 102417 Operation of a Family Child Care Home. An exit interview was conducted, a copy of this report, Appeal Rights, and Notice of Site Visit were provided to Licensee Sarahyah Yisrael. The licensee was reminded that the Notice of Site visit must remain posted for 30 consecutive days and violation of this regulation may result in civil penalties.

In addition, four out of nine sleeping cots were damaged and dirty which poses a potential health, safety and personal rights risk to children in care.

Safety precautions shall include but not be limited (4) Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children. This requirement was not met as evidenced by: Based on observation, LPAs observed the door to the cabinet under the kitchen sink to be broken, holes in front metal security door and a bottle of lighter fluid in the front yard which poses a potential health, safety and personal rights risk to children in care.
