102417 (d) Operation of a Family Child Care Home (d) The home shall provide safe toys, play equipment and materials this requirement has not been met as evidenced by: Based on observation, LPAs observed multiple toys that were dirty and broken. In addition, four out of nine sleeping cots were damaged and dirty which poses a potential health, safety and personal rights risk to children in care.

102417(g)(4) Operation of a Family Child Care Home (g) The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not be limited (4) Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children. This requirement was not met as evidenced by: Based on observation, LPAs observed the door to the cabinet under the kitchen sink to be broken, holes in front metal security door and a bottle of lighter fluid in the front yard which poses a potential health, safety and personal rights risk to children in care.

Criminal Record Clearance (d) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing, or volunteering in a licensed facility:The requirement is not met as evidenced by: The Licensee has 2 adult children who are not fingerprint cleared and associated to the facility Personnel Summary Report, which poses an immediate health, safety or personal rights risk to children in care.

AB 1207–Mandated Child Abuse Reporting: Child Day Care Personnel Training, beginning January 1, 2018, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. The requirement is not met as evidenced by:Based on records review, LPA Wilburn observed Licensee's Mandated Reporter certificate expired on 3\9\21, which poses a potential health and safety hazard.

PERSONNEL REQUIREMENTS
