TEACHER QUALIFICATIONS AND DUTIES
Based on observation and record review, the licensee did not comply with the section cited above. In touring the facility LPA observed Staff #5, alone with children and in reviewing their file, it was determined that they were missing the required Child, Family, and Community course. This poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. In reviewing staff files, LPA determined that Staff #3 did not have the required health screening/TB test on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

In conducting children record reviews, LPA observed an admission agreement for Child #8 to be in the infant/toddler program however, the child was in a Pre-K classroom. There was no updated Admission agreement to reflect the change. This poses/posed a potential health, safety or personal rights risk to persons in care.
