What Happened

Incident investigation into Unusual Incident Report documenting staff member physically mishandling children during activity time. Staff member picked children up and tossed them back onto activity rug, resulting in injury and child limping. Staff was terminated. Type A citation issued.

Observations

  • Staff member observed picking up children off floor and tossing them back onto activity rug
  • One child sustained leg/hip injury from being tossed, resulting in visible limp when walking
  • Staff member responsible was terminated
