Follow-up on corrections·April 2, 2025

On 4/2/2025, Licensing Program Analyst (LPA) Samuel Lopez arrived at the facility to conduct an inspection for a separate and unrelated reason. During the inspection, LPA addressed and verified information previously obtained that there was an emergency evacuation on March 26, 2025, of the entire grounds that included staff from Church, Child Care facility and children in attendance. The evacuation was due to a bomb threat reported. About an hour later, an all clear was given after local government agencies conducted a check and verified it to be a false alarm. Although there were no injuries to report, the nature of the incident is required to be reported to the Riverside Child Care Regional Office, as an Unusual Incident Report (UIR). The facility failed to report the incident as required by Title 22 regulations. LPA was also informed that the Director was on vacation during the day of the incident and the staff present at the facility were not aware of licensing reporting requirements. Based on the information gathered, facility failed to comply with regulations regarding Reporting Requirements and Child Care Center Directors Qualifications and Duties. See LIC809-D for cited deficiencies. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Director Deborah Angulo.
