What the inspector wrote

On the above noted date and time, Licensing Program Analyst (LPA's), Griselda Castellon and Diana Brasel conducted an announced 2nd Pre-Licensing inspection for a New Single License. Upon arrival, LPA's met with Azizi Gates Applicant and Laura Thompson Director. On 01/23/2025 Azizi Gates at time of visit requested to withdraw the original infant request, a written withdrawal request was obtained. Ms. Gates requested to move forward with the preschool license only. On 01/29/2025 Ms. Gates sent an email requesting to included the original infant component along with the preschool component. The request is to be licensed for 36 preschool children ages 2-5 in the Orange Room, Green Room, and Blue Room, and 5 infants ages 6 weeks - 2 years in the in the Purple Room, with 3 cribs. An updated LIC 200A was obtained during this visit to show the updated request. The Hours of operation will be Monday through Friday 7:00 am to 6:00 pm. A granted fire clearance was obtained on 08/27/2024, which included both preschool and infant. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Ms. Gates and Ms. Thompson that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Ms. Gates stated they will be locked in the office cabinet. A fully equipped first aid kit is located in the kitchen area. There is an operational carbon monoxide detector on site located outside of the kitchen area. All required licensing documents were observed posted in the lobby. Children will be signed in and out using the Bright Wheel app provided to parents. LPA's discussed with applicant that a hard copy would be required if the system was down. LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured for the preschool component is 1,276.97 sq ft, which is sufficient to accommodate the requested capacity. Total indoor activity space measured for infant component is 286.52 sq ft, which is sufficient to accommodate the requested capacity. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, cots, bookshelves, and other activity supplies for the children. Drinking water will be available in the classrooms via the children's personal water bottles. The facility has a Brita filtered water dispenser and disposable cups. LPA's observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. LPA's observed a total of 3 sinks and 3 toilets available for preschool children's use, which is sufficient to accommodate the requested capacity of 36 children. LPA's observed one children's sink for hand washing, one changing table with an adult sink. Per Ms. Gates one potty chair will be purchased for the infants, which is sufficient to accommodate the requested capacity of 5 children. (see correction needed) There is a separate staff restroom equipped with a toilet and a sink in the staff lounge next to the kitchen. The isolation area for children who are ill will be in the lobby area near the directors desk The facility currently has a fully fenced playground area for the preschool and infant component, which is a wrought iron fence with mesh, the fencing surrounding the property is at least four feet high. The fencing separating the preschool and infant playground measured 3' 11", which is not sufficient. The fencing separating the preschool and infant playgrounds shall be 4 feet. (see correction needed) The total square footage for all the outdoor activity space for the preschool component is 894.88 sq ft, which is only sufficient for 11 children at one time, which is insufficient to accommodate the requested capacity of 36 preschool children. The total square footage for all the outdoor activity space for the infant component is 301.89, which is only sufficient for 4 children at one time, which is insufficient to accommodate the requested capacity of 5 infant children. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure for both components. Shade is provided via two wind sails for the preschool playground and a built in gazebo for the infant playground. The side of the building will provide shade for both playgrounds. There are sufficient outdoor age-appropriate toys and play equipment available on the playgrounds. There is a climbing structure on the preschool playground which is age appropriate for the groups that will use it. All equipment is properly anchored. There is adequate cushioning in fall zones of the climbing equipment via fit lock rubber tiles. The infant playground does not have sufficient cushioning material. (see corrections needed) Drinking water is available outdoors via the children's personal water bottles. LPA's observed some hazards on both the preschool and infant playground. (see corrections needed) The owner/administrator was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Facility will provide morning snack and afternoon snack. The parents/caregivers will provide lunch. The kitchen area currently includes refrigerator, freezer, one microwave, and sink. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food is observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via upper cabinets in the kitchen. LPA's observed 2 small high chairs without the locking feeding trays, they have broad-based legs, and plastic seats that are in good repair. (see correction needed for the locking trays) The high chairs are made of washable, moisture-resistant material. Changing table has at least 1\342\200\235 padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3\342\200\235 high and the changing table is within arm\342\200\231s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via a wood wall with two plexiglass window areas. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs, Needs and Services Plans, Sleep Plans will be kept in the white cabinet. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. APPLICANT WILL WAIT UNTIL FACILITY OPENS TO DETERMINE IMS NEEDS: Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. MyChildCarePlan.org--Child Care Centers and Family Child Care Home: Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Subscribe to CCLD important information - Child Care Centers and Family Child Care Homes: Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. LPA's provided and reviewed the LIC 311A on 01/23/2025. The following corrections are needed prior to the issuance of the license: 1. Blue Room: Proof that the chipped paint on the entrance door and door frame have been repaired. 2. Preschool Playground: All chipped concrete that poses a tripping hazard shall be repaired. 3. Purple Room (infant): Proof that the locking trays for the two small highchairs have been installed. Proof that the discussed potty chair has been purchased and obtained. 4. Infant Playground: Proof that the fence separating the two playgrounds measures 4" tall. Proof that cushioning material has been installed on at least one half of the playground. All chipped concrete that poses a tripping hazard shall be repaired. 5 . The discussed documents for the director packet shall be submitted. Exit interview conducted and report was reviewed with the applicant Azizi Gates. Applicant understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. A license for 36 preschool children and 5 infant children as a single license will be granted upon a final file review and the approved granted waivers needed for both playgrounds.
