What the inspector wrote

Licensing Program Analyst (LPA) Kelly Ferrara met with Assistant Director Nancy Galvan for an unannounced annual inspection on September 3rd, 2021. At 9:15 AM, LPA toured the facility including all activity and classroom spaces, restrooms, food service and outdoor play areas. Appropriate ratio and care and supervision were observed during today's inspection. Census included 46 preschool children being supervised by seven staff. Director was reminded never to exceed the conditions, limitations and capacity specified on the license. Facility hours of operation are Monday through Friday from 6 AM to 6:30 PM.. Classrooms all appeared clean including the carpets and floor. Chemicals were kept inaccessible to children in a high cabinet. Playground equipment and surfaces were inspected and are in good condition with enough resilient material under climbing structures to absorb a fall. Uncontaminated drinking water was readily available to children both indoors and outdoors. Bathrooms were clean and all sinks and toilets were in operating condition. There are no firearms or bodies of water on the premises and the facility has at least one functioning smoke and carbon monoxide detector. Menus were posted for AM and PM snack and the children are provided lunches. LPA observed all required licensing documentation was posted for parental review. LPA observed that parents are signing their children in and out of the center electronically. LPA discussed current Covid guidelines with the Director. At 11 AM, LPA reviewed staff transcripts and observed that an Aide without any completed coursework was in a classroom with a qualified teacher and both were supervising a group of 20 children. At least one staff had a current Pediatric CPR and First Aid certification that expires August 2023. At 11:30 AM, LPA observed that five out of 14 total files were missing a current Mandated Reporter certificate and six out of 14 total files were missing a Health Screen report (LIC503). At 12:30 PM, LPA observed a sample of preschool children's files. LPA observed that four out of eight files were missing the Physician's Report (LIC701) and seven out of eight files were missing the Immunization record (CDPH286). LPA observed a current children's roster and a fire/disaster drill log. Each classroom is equipped with First Aid equipment. Continued on 809-C Incidental Medical Services (IMS) are provided by this facility. For additional IMS information, Director was advised to see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm . The facility evaluation report was reviewed and discussed with the Director. Director was encouraged to the visit the department's website at WWW.CCLD.CA.GOV for information regarding child care updates, PINs, forms, regulations and legislation pertaining to child care centers. A notice of site visit was provided and should remain posted for a period of 30 days for parental review. See page 809 D for citations issued during today's inspection.

    (b) The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance.This requirement was not met as evidenced by: LPA observed a staff with no college coursework supervising a group of 20 children with a qualified teacher. This is a potential health and safety risk to children in care.

    All personnel... shall be in good health and shall be physically and mentally capable of performing assigned tasks.1)...good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician. This requirement was not met: LPA observed 6/14 staff files missing LIC503 Health Screen.

    (a) A separate, complete and current record for each child is maintained in the child care center. This requirement was not met as evidenced by: Children's files were incomplete and missing forms LIC701 and CDPH286. This is a potential health and safety risk to children in care.

    ... licensed child day care facility shall complete the mandated reporter training...and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement was not met: LPA observed 5/14 staff files missing a current Mandated Reporter certificate.
