    HEALTH-RELATED SERVICES

    (a) The licensee shall immediately notify the child's authorized representative if the child becomes ill or sustains an injury more serious than a minor cut or scratch. The licensee shall obtain specific instructions from the authorized representative regarding action to be taken. This requirement was not met as evidenced by witness statements and documentation that, when a child became injured in care, staff failed to provide authorized representative with an incident/injury report. This is a potential risk to health, safety and or personal rights of children in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time ... Supervision shall include visual observation. This requirement was not met, as evidenced by: witness statements and photographic evidence that C1 did sustain injury in care. This is a potential risk to health, safety and or personal rights of children in care.

  • CRIMINAL RECORD CLEARANCE

    Based on record review, the licensee did not comply with the section cited above for staff Grace Cook which poses an immediate health, safety or personal rights risk to persons in care.

  • REPORTING REQUIREMENTS

    Reporting Requirements (d) Upon the occurrence… a report shall be made to the Department… a written report containing the information…shall be submitted to the Department within seven days…(1)Events reported shall include the following: (B) Any injury to any child that requires medical treatment. This requirement was not met as evidenced by: LPA learned of an injury to a child that occurred at the center that was not reported to Licensing. This poses a potential health and safety risk to children in care.

    (b) The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance.This requirement was not met as evidenced by: LPA observed a staff with no college coursework supervising a group of 20 children with a qualified teacher. This is a potential health and safety risk to children in care.

  • PERSONNEL REQUIREMENTS

    All personnel... shall be in good health and shall be physically and mentally capable of performing assigned tasks.1)...good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician. This requirement was not met: LPA observed 6/14 staff files missing LIC503 Health Screen.
