What Happened
During a case management inspection, analysts found school-age children placed in a preschool licensed classroom with preschool children, creating an immediate health and safety risk. File review confirmed the children’s ages and enrollments, and a deficiency was cited.
Observations
- There were 57 children and 8 staff present at arrival.
- Seven school-age students and two preschool children were observed in a preschool licensed classroom.
- File review verified the ages and enrollments of the children involved.
- A Plan of Correction was reviewed and developed with the substitute director.
