What Happened
This case management deficiencies visit stemmed from a complaint investigation and record review. The inspector found that a teacher injured a child by forcefully pulling off the child's backpack and that the incident was not reported; interviews also showed the teacher often yelled at children inappropriately, leading to Type A and Type B citations.
Observations
- Record review showed a February 2024 incident involving a teacher pulling a child's backpack off forcefully and causing injury
- The teacher was placed on administrative leave on March 5, 2024 and terminated on March 22, 2024
- The injury incident was not reported to Community Care Licensing
- Interviews revealed the teacher often yelled at children inappropriately
- A Type A citation for personal rights and a Type B citation for reporting requirements were issued

101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights:...(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... This regulation was not met as evidenced by: LPA learned T1 forcefully pulled off a backpack from a child, causing injury. Interviews revealed that T1 would inappropriately yell at children in care. This poses an immediate health, safety, or personal rights risk to children in care.
