What does Type A vs Type B mean?
Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

(7) The changing table and changing pads shall be disinfected after each use even when disposal covers are used. This requirement was not met, as evidenced by: Based on observation and interview, changing table mats in the facility had cloth covering with disposal covers. Staining and dirt were observed under mat. This poses a potential risk to the health and safety of children in care.

Initials of staff person whoconducted each check. This requirement was not met, as evidenced by: Based on interview and observation, 15 minute checks of napping infants were not documented. This poses a potential risk to the health and safety of children in care.
