INFANT CARE CENTER SLEEPING EQUIPMENT

Based on observation, the licensee did not comply with the section cited above by having an infant napping with the blanket in a crib, which poses an immediate health, safety or personal rights risk to persons in care.

101416.5 Staff-Infant Ratio (b) There shall be a ratio of one teacher for every four infants... (1) An aide may be substituted for a teacher when... : (B) Each aide is responsible for... supervision of... no more than four infants. This requirement was not met as evidenced by: Based on interview the facility did not comply with the above regulation as seven children in the toddler room were temporarily supervised by the Director alone on a short-staffed morning which poses Health, Safety, or Personal Rights risk to persons in care.

101171 Fire Clearance (a) All child care centers shall secure and maintain a fire clearance approved by the city or county fire department, the district providing fire protection services, or the State Fire Marshal. This requirement was not met, as evidenced by: Based on interviews, the facility did not comply with the above regulation as it was learned that the fire alarm system is not working and the facility failed a previous inspection from the fire marshal.

CRIMINAL RECORD CLEARANCE

101170 Criminal Record Clearance (e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility:...(2) Request a transfer of a criminal record clearance as specified in Section 101170(f).. This requirement was not met, as evidenced by: Based on interview and file review, 2 staff in the infant room (Wijdan Alsubaihawi, Laura Scott) were not associated to the facility.

101438.1 Infant Care General Sanitation (c) Washing, cleaning and sanitizing requirements for areas used by staff with infants, or for areas that infants have access to, are as follows: ...(2) Carpeted floors and large throw rugs that cannot be washed shall be vacuumed at least daily and cleaned at least every six months, or more often if necessary. This requirement was not met, as evidenced by: Based on observation, there were stains on the carpets in the infant facility. This poses a potential risk to the health and safety of children in care.

BUILDINGS AND GROUNDS

Based on observation, the licensee did not comply with the section cited above as several areas of the wall were chipping, a hole was observed on a door, a counter laminate was peeling, which poses/posed a potential health, safety or personal rights risk to persons in care.

INDOOR ACTIVITY SPACE FOR INFANTS

Based on observation, the licensee did not comply with the section cited above preschool children were observed in the toddler classroom for the day, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above as there was not a wall divider separating the napping space, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above as changing mats had cloth covering, which poses/posed a potential health, safety or personal rights risk to persons in care.

INFANT NEEDS AND SERVICES PLAN

Based on observation, interview, and record review, the licensee did not comply with the section cited above as LIC9227 forms were not observed in infant files, which poses/posed a potential health, safety or personal rights risk to persons in care.

(7) The changing table and changing pads shall be disinfected after each use even when disposal covers are used. This requirement was not met, as evidenced by: Based on observation and interview, changing table mats in the facility had cloth covering with disposal covers. Staining and dirt were observed under mat. This poses a potential risk to the health and safety of children in care.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION FOR INFANTS

(B) Staff shall physically check on sleeping infant(s) every 15 minutes and document the following....Documentation shall include: 1.Date. 2.Infant’s name.3.Time of each 15-minute check.4. Initials of staff person whoconducted each check. This requirement was not met, as evidenced by: Based on interview and observation, 15 minute checks of napping infants were not documented. This poses a potential risk to the health and safety of children in care.

(D) An infant's head shall not be covered while sleeping. This requirement was not met, as evidenced by: Based on observation, napping children in the infant classroom had their head's covered with a blanket. This poses an immediate risk to the health and safety of children in care.

(e) Each infant's bedding shall be used for him/her only. Such bedding shall be replaced when wet or soiled, or when the crib, mat or cot is to be occupied by another infant. (1) Bedding shall be changed daily, or more often if required by Subsection (e) above. This requirement was not met, as evidenced by: Based on observation and interview, Infant bedding was not changed daily. This poses a potential risk to the health and safety of children in care.

INFANT CARE TEACHER QUALIFICATIONS AND DUTIES

Infant Teacher Qualifications and Duties (b) Prior to employment, an infant care teacher shall have completed, with passing grades, at least three postsecondary semesters or equivalent quarter units in early childhood education or child development, and three postsecondary semester or equivalent quarter units related to the care of infants, at an accredited or approved college or university. This requirement was not met, as evidenced by: Based on observation and record review, S1 and S2 were left alone with children in care and do not have ECE units. This poses an immediate risk to the health and safety of children in care.
