The complaint investigation substantiated a lack of visual supervision in both indoor and outdoor areas. During the visit, one child in the classroom was not within staff's visual supervision while another child was being diapered, and later two children entered the infant play yard and moved out of a teacher's view. Deficiencies were cited based on these observations.
