What the inspector wrote

Licensing Program Analyst (LPA) Erwina Pascual-Golamco, met with Licensee (L), Kelli Vaccaro, for a case management inspection. The purpose of the inspection was explained. During a recent investigation, LPA learned of an incident at the facility in May 2025, when a child was left in the infant/toddler playground and did not have direct visual observation and supervision by a staff person. According to Director, incident was approximately two minutes. LPA\342\200\231s interviews revealed that the incident was not reported to the department and to authorized representative/s. Title 22 Deficiency has been issued on the attached LIC809-D page. This report was reviewed with Licensee, Kelli Vaccaro, and an exit interview was conducted. Appeal Rights and a Notice of Site Visit was provided to L, who will post it for a period of 30 days for parental review.

    101429 Responsibility for Providing Care and Supervision for Infants (a) In addition to Section 101229, the following shall apply: (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement is not met as evidenced by: Based on multiple witness statements and interviews, a child was left in the infant/toddler playground when the group and staff responsible for the child went back to their classroom from outdoor play and according to Director, child was out of sight for approximately two minutes, and another staff member from adjacent preschool playground saw the child and brought the child back to their specific group, which poses an immediate health, safety, or personal rights risk to persons in care.
