101429 Responsibility for Providing Care and Supervision for Infants (a) In addition to Section 101229, the following shall apply: (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement is not met as evidenced by: Based on interview, and record reviews, Director (D) did not comply with the section cited above as D self reported to the dept. and submitted an Unusual Incident Report (UIR) that on 06/23/25, a child was left in the playground without staff supervision for approximately under a minute, which poses an immediate health, safety, or personal rights risk to persons in care.

This requirement is not met as evidenced by: Based on multiple witness statements and interviews, a child was left in the infant/toddler playground when the group and staff responsible for the child went back to their classroom from outdoor play and according to Director, child was out of sight for approximately two minutes, and another staff member from adjacent preschool playground saw the child and brought the child back to their specific group, which poses an immediate health, safety, or personal rights risk to persons in care.
