What the inspector wrote

Licensing Program Analyst (LPA) Erwina Pascual-Golamco, met with Current Facility Representative (CFR), Ashley Olivine, for a case management inspection regarding a self reported incident on 05/16/25. This is a subsequent visit from 05/30/25. Purpose of the inspection was explained. LPA toured the facility, including all activity and classroom spaces. CFR was reminded never to exceed the conditions, limitations, and capacity specified on the license. CFR self reported and submitted a written Unusual Incident Report LIC624, that on 05/16/25, around afternoon pick up time, a child was found in the bathroom by a staff member without supervision. According to LPA's gathered information and interviews, child was alone for approximately one minute. Staff member informed previous facility representative regarding the incident. CFR self reported the incident to the department, interviewed staff involved, and took necessary steps/actions regarding incident. Previous facility representative informed parent/guardian of child regarding incident. Based on interviews and record review, Title 22 Deficiency has been issued on the attached LIC809-D page. The CFR was informed that this report dated on 06/12/25 documents two Type A citations. An immediate civil penalty is being assessed in the amount of $1000. CFR shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. Exit interview was conducted with CFR, appeal rights were provided, and A notice of site visit was given CFR, who will post it where visible to parents/guardians for 30 days. A signed Acknowledgement of Receipt of the Licensing Report (LIC9224) must be placed in the child's file for verification.

This requirement is not met as evidenced by: Based on interviews, and record review, Current Facility Representative (CFR) did not comply with the section cited above as CFR self reported to the dept. and submitted an Unusual Incident Report (UIR) that on 05/16/25, a child was in the bathroom without staff supervision for approximately one minute, which poses an immediate health, safety, or personal rights risk to persons in care.
