This requirement is not met as evidenced by: Based on interviews, and record review, Current Facility Representative (CFR) did not comply with the section cited above as CFR self reported to the dept. and submitted an Unusual Incident Report (UIR) that on 05/16/25, a child was in the bathroom without staff supervision for approximately one minute, which poses an immediate health, safety, or personal rights risk to persons in care.
