What the inspector wrote

On Wednesday, May 6, 2026, Licensing Program Analyst (LPA) Lea Habtom arrived at the facility for an unannounced inspection to close a complaint. There were 3 children consisting of 1 infant, 1 preschool and 1 of licensee's own school age child being supervised by the licensee and her spouse. During the investigation that was conducted by Investigator Meagan Hammond, it was mentioned on a few occasions that day care and licensee's own children utilized the garage during day care hours. The garage is restricted based on the fire clearance and is an off limits area and should not be used during day care hours. Based on the information gathered, a Type A deficiency was issued. LPA Lea Habtom informed licensee Khalida Nazrabi that this report dated May 6, 2026, documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the licensee Khalida Nazrabi to provide a copy of this licensing report dated May 6, 2026, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. This report was reviewed with the licensee, Khalida Nazrabie. A notice of cite visit was provided to be posted for 30 days. Appeal rights provided.

    102416.3(a)(6) Alterations to Exisiting buildings: Prior to making alterations or additions to a family child care home or grounds, the licensee shall notify the Department of the proposed changed, including, but not limited to, the following: (6) Any change from an area of the family child care home previously identified as "off limits" to an area where care and supervision will be provided to children in care. This requirement was not met as evidenced by child interviews that disclosed the garage is being used for day care children which is an immediate health and safety threat to the children in care.
