What the inspector wrote

Licensing Program Analysts (LPAs) Loraine Perez and Andrea Cortez met with Fatmata Pearson, Licensee, for a case management deficiencies inspection. LPAs made observations, conducted interviews and collected relevant documentation. Licensee requested to update off-limit areas at the time of inspection. Off-limit areas are now primary bed and bathroom, and bedroom 1, laundry room and garage. There is a census of 10 children, 1 infant, 7 Preschool, 2 school age. Present is Licensee and one assistant. LPAs conducted a tour of the facility and it was observed an infant was asleep in a bedroom with the door closed. There is an assistant that has not been associated to this facility. Civil penalty assessed on LIC809D. Title 22 Deficiencies have been cited on the attached LIC 809D . Upon receipt of Type A citations, facility shall post and provide copies of the LIC 809D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Reports LIC 809D in each child's files. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Fatmata Pearson.
