What the inspector wrote

This is an amended report. On 6/4/2025 at approximately 9:45 AM, Licensing Program Analyst (LPA) Pa Dao Vang met with Assistant Director, Kaeleigh Dizon, and Licensee/Director, Jessica Dawson-Brown, for a Case Management inspection regarding an Unusual Incident Report submitted the Sacramento Regional office on 5/21/2025. Upon arrival, LPA observed three infants, four toddlers, and nine preschoolers supervised by six staff. During today's inspection, LPA toured the facility, made observations, conducted interviews, and inspected the classroom and outdoor spaces. The Assistant Director stated the floors of the outdoor yards are made out of European tiles. At 10:20 AM, most of the tiles in the yard were in direct sunlight and were hot to the touch as LPA put her hands on the different areas. This included the entire side of the yard, in which the Toddler area is located. LPA went out again at 11:36 AM and observed a little shade covering the Toddler side, but the majority of the tiles were still in direct sunlight. At 1:33 PM, LPA went to the yard and observed more shade covering the entire yard. According to the Assistant Director, the incident took place outside on 5/21/2025 around 10:45 AM in the Toddler yard. There were a total of six children and two staff present outside during the incident. The Assistant Director walked over to the sink to wash a ball for Child 1 (C1). C1 was sitting upright on the tiles near the corner of the yard. C1 then started to crawl on the tiles and cried. Both staff heard the crying and picked C1 up immediately from the tiles. They noticed there were sheets of skin peeling off of C1\342\200\231s palms, forearms, and an injury on the nose. Staff 1 (S1) stated there was no shade in the tile area where C1 got injured. Parent was notified immediately and took C1 to the hospital. Continue on LIC809-C... This is an amended report. Parent also stated C1 sustained second degree burns on right and left palms and forearms. The Department is assessing a Civil Penalty in the amount of $10,000 for the serious injury C1 sustained while in care. Based on LPA observations and interviews conducted, a deficiency is being cited on page LIC809-D in accordance with the California Code of Regulations, Title 22. LPA informed the Assistant Director, Kaeleigh Dizon, that this report dated 6/4/2025, documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed the Assistant Director to provide a copy of this licensing report dated 6/4/2025, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. This report and appeal rights were reviewed and provided to the Assistant Director. A notice of site visit was also provided to be posted for 30 days.
