Based on observation, the licensee did not comply with the section cited above. LPA observed lifted cement in the play area that can be a potential tripping hazard, a piece of wood on the railing of the stairs that is broken and can injure children, siding of building is coming off, a swing that is broken, outdoor chairs are cracked and cause a potential hazard and fence is falling apart which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed one teacher in room 1 and 15 children with one teacher in room two. Facility was out of ratio which poses an immediate health, safety or personal rights risk to persons in care.
