What the inspector wrote

On 10/3/2025, at 1:35PM, Licensing Program Analyst (LPA) Liridon Fici- Doni conducted an unannounced Case Management inspection regarding an incident that occurred on 9/25/2025. LPA met and was greeted by Hernandez, Angela- Director. LPA informed the Director the reason for visit and gathered census of children during today's visit. During today's visit, LPA interviewed the director, along with two (2) children who witnessed the incident. On 9/25/2025, the Director informed LPA that she did not personally witness the incident; however, Staff 2 (S2), who was the driver involved, submitted a written statement describing what transpired. According to the Director, the incident occurred at Cerra Vista Elementary School during student pick-up. S2 was the second driver in line, waiting for a truck to leave so she could safely move forward and park. While S2 was waiting, the driver of a Fusion bus drove around her vehicle, cut in front of S2, and attempted to take her position in line. The Director stated that the Fusion bus driver became upset, exited the vehicle, approached S2\342\200\231s bus, and began banging on the window while yelling, \342\200\234You cut me off!\342\200\235 The Director reported that the incident was witnessed by school yard duty staff and two parents present at pick-up. After the incident, S2 returned to the center and immediately informed the Director. The Director stated she contacted both the school principal and the children\342\200\231s parents to report what had occurred. The principal later informed the Director that a yard duty staff member had provided a verbal statement confirming that the Fusion bus driver was at fault in the incident. When interviewed, both children stated that the driver of the Fusion bus cut in front of S2\342\200\231s bus and began hitting on S2\342\200\231s window. No Deficiencies were issued at this time. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview conducted with director, and a copy of this report was review and provided along with appeal rights.
