During the investigation LPA toured the entire facility and did not observe commingling of children in care. Staff members interviewed stated they were aware of the commingling waiver in place and its specific guidelines. All staff interviewed confirmed the facility does not commingle children outside of the specific stated times. LPA’s direct observations and statements from those interviewed do not support the allegations that children are being commingled. This agency has investigated the complaint alleging children are being commingled. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. AT THE TIME OF VISIT NO DEFICIENCIES WERE CITED A NOTICE OF SITE VISIT WAS GIVEN. STAFF WAS INSTRUCTED TO POSTED IT IN A PROMINENT LOCATION AT THE FACILITY. THE STAFF UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS. An exit interview was conducted, A copy of this report and appeal rights were given to Staff during this inspection on 07/09/2021.
