Licensing Program Analyst (LPA) Rachel Zeron made an unannounced case management visit to conclude the investigations initiated by unusual incident report received by the Riverside Regional Child Care Office. LPA met with William Alvarez and explained the reason for the visit. The dates of the incident reports investigated by LPA Zeron were 06/23/22, 07/12/22 and 07/21/22, all incidents were pertaining to personal rights of children in care. During the course of the investigation, interviews conducted with pertinent individuals concluded with conflicting information obtained. Therefore, the incidents that occurred on 06/23,07/12 and 07/21/2022 are unsubstantiated at this time. Although the allegations may have happened or are valid, there is no preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are deemed UNSUBSTANTIATED at this time. No deficiencies were cited. An exit interview was conducted, and a copy of this report and a Notice of Site Visit (required to be posted for the next 30 days) was provided to the Director William Alvarez on 09/29/2022. LPA verified the Notice of Site Visit was posted in a prominent location before leaving the facility.
