This requirement was not met as evidenced by: The center failed to report the incident of Child#1 running out of the facility on 03/21/24 to community care licensing (CCL) on a timely manner.This is an potential Health and Safety Risk for the children in care.

  • OBSERVATION OF THE CHILD

    101226.3(b) Observation of the Child. Any unusual behavior, any injury or signs of illness requiring assessment and/or administration of first aid by staff shall be reported to the child's authorized representative and recorded in the child's record. This requirement was not met as evidenced by: Per files review conducted by LPAs during inspection 04/04/24 and 04/26/24 no ouch/incident(s) reports related behavior nor injuries were documented for Child#1.

Each child shall be accorded safe, healthful and comfortable accommodations, furnishings and equipment.This requirement was not met as evidenced by: There were multiple incidents documented in a 1-3 month period involving the same child(ren) hitting other children. Facility failed to protect the safety of the children. This is an immediate Health and Safety Risk for the children in care.

    (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below....This requirement is not met as evidenced by: Per LPAs observations and Staff own admission facility has been out of ratio approximately 3-5 times during 03/23 and 04/2023. In addition, Upon arrival to the facility LPA Ruiz observed 15 children in the multipurpose room (Classroom N.61) under the supervision of Staff N.1. Few minutes later Staff N.2 arrived to remove additional children and they were taken to Classroom N.93 with Staff N.3. "This is an immediate Health and Safety risk for the children".

  • TEACHER QUALIFICATIONS AND DUTIES

    b) Prior to employment, a teacher shall meet the requirements of (b)(1) or (b)(2) below: (1) A teacher shall have completed,...at least six post-secondary semester or equivalent quarter units of the...requirement specified...below, or shall have obtained a Child Development Assistance Permit...This requirement is not met as evidenced by: Per interviews conducted and staff own admission, it was confirmed that Staff N.1 was observed working in a classroom as a lead teacher providing care and supervision. However, staff records revealed that Staff N.1 has college credits; however, courses in transcripts do not qualify Staff N.1 to work as a qualified Lead Teacher.
