What the inspector wrote

Licensing Program Analyst (LPA) Taityana Benson conducted an unannounced visit to the facility for the purpose of following up on the submission of an Unusual Incident Report (UIR) that was submitted by the facility. The initial UIR was received by the licensing agency on 07/18/2025. The UIR documented an incident involving a day-care child. LPA was granted entry into the facility and met with Director Michelle Martinez to discuss the purpose of the visit and conducted a tour of the facility to obtain census. The UIR received by the licensing agency documented a day-care child sustained an injury on 07/16/2025, while in care at the facility. Facility records were reviewed, documentation was obtained, and interviews were conducted with pertinent parties. It was disclosed that on 07/16/2025 while day-care children were in the classroom, a staff member was present and witnessed a day-care child (1) shove another day-care child (2). It was disclosed the staff member begin approaching the day-care child (1) to assist with redirection prior to the incident. However, before the staff member could prevent the incident from occurring, the other day-care child (2) fell forward an sustained an injury to their face from a piece of furniture. It was disclosed that a staff member immediately provided comfort to the day-care child (2), examined the injured area, and applied first aid to the injury. The LPA inspected the furniture inside of the classroom and observed it to be in good repair. Pertinent parties stated the facility has adequate staffing and supervision of day-care children in care is always maintained. It was discussed that the facility followed their operational plan by documenting the incident, notifying both of the day-care children's parents, and provided parents with a electronic report. LPA has observed the facility to adhere to written policies and procedures regarding supervision, documenting incidents, classroom behavior management and redirection. Lastly, the facility installed edge protectors on furniture to cushion day-care children's contact. Report Continued On LIC809-C No deficiencies cited at this time. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and the report was reviewed the Director Michelle Martinez.
