No child(ren) shall be left without the supervision of a teacher at any time...and Supervision shall include visual observation. This regulation was not met as evidenced by: Based on observation, surveillance video, interviews, and record review, the facility staff failed to provide care & supervision necessary while a child in care obtained an injury that resulted in medical attention, which poses an immediate health and safety risk to persons in care.

This ia an immediate health, safety and personal rights risk to children in care.
