What the inspector wrote

A case management inspection is being conducted in response to the receipt of an Unusual Incident Report (UIR) from the facility. Licensing Program Analysts (LPAs) Blanca Ruiz and Rachel Zeron met with the Director, Megan Peveler to follow up on the UIR submitted to the department. Facility was toured and census was taken. Director explained that on 04/15/24 at approximately 12:00 p.m. Infant Classrooms A and B were transitioning to nap, Staff #1 gave Staff #2 the bottle for Child#2, per parent request to assist the infant to go to sleep as child is learning nap without the bottle. Staff #2 then observed that the bottle given belong to Child#1 in the infant Classroom A and as Child#2 attempted to take the bottle to his/her mouth. Staff #2 intercepted the bottle and the child did not consume any of the liquid in it. Parents of infants have been informed of the incident. Per facility review, teacher to child ratio was 1:4. Mistake occurred due to exception allow at the center for Child#2. During this inspection, LPAs verified infant bottle to be properly labeled and dated. After the reported incident, infant in Classroom B is not longer requiring the bottle to sleep. Per facility director, Staff had a discussion regarding the incident to prevent irregularities with infants food. Based on information gathered, the facility acted appropriately, and no violations have been identified. An exit interview conducted, and report was reviewed with licensee, Megan Peveler. Appeal rights were discussed, and A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC, UPON THEIR REQUEST, FOR THREE YEARS.
