Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time ... This requirement was not met as evidenced by: In or about June 2021, there had been at least two different occasions that children were left unsupervised while staying in the front office area waiting for additional teaching staff to arrive to work in the morning. The children were in the hallway around the corner of the front counter and in the staff room without any staff member physically present with them in the same area. That presented immediate risks to the children's health and safety.

This requirement is not met as evidenced by: In or about June 2021, due toinsufficient teaching staff to maintain proper teacher-child ratio in the class, some children were made to stay in the front office, staff room and the director's office with office staff for extensive period of time. That was a violation of the children's personal rights as those areas are not licensed as part of the child care activity areas and not age-appropriate for the children.
