    101229 Responsibility for Providing Care and Supervision (a)The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time…This was not met as evidenced by: Based on documentation and interview, a child was left alone for about 15 minutes in the classroom which is an immediate health, safety or personal rights risk to children in care.

    Based on record review and interview, the licensee did not comply with the section cited above in that Staff 2 (S2) and Staff 5 (S5) did not complete renewal of madated reporter training every 2 years. S2's mandated reporter training needed to be renewed by 8/27/2025 and S5's mandated reporter training needed to be renewed by 10/20/2025. This poses/posed a potential health, safety or personal rights risk to persons in care.

    a) Prior to construction or alterations, the licensee shall notify the Department of the proposed change(s).This requirement was not met as evidenced by: Per Director, Alisa Holtegarrd, center playground renovation/construction began 09/07/23; this will continue throughout the other playgrounds at the center one at a time until all play areas are completed. CCL was not informed of construction changes being done. "This poses an potential Health and Safety risk for the children in care".

(1) No child(ren) shall be left without the supervision of a teacher at any time...This was not met as evidenced by: Based on observation and interviews, LPA's observed two children using the bathrooms unattended. The door to the bathrooms were closed when the children were inside the bathrooms, each child entering the bathroom by themselves, while the teacher stayed outside in the classroom. which posed a potential Health, Safety, and Personal Rights risk to the subject child and potential children in care.

    Based on interview, record review, the licensee did not comply with the section cited above in by admission stated the facility kitchen outlets and water fountains used for water consumption and food preparation have not been tested for possible lead exposure, which poses a potential health, safety or personal rights risk to persons in care.

(1) Supervision shall include visual observation.This requirement has not been met as evidenced by: Based on Interviews and records reviewed, on 08/04/22 Staff were transitioning between scheduled breaks, in this timeframe a child was struck multiple times and kicked by another child and as a result, sustained a fractured ankle before staff intervention. This poses an immediate health and safety risk to the children in care.

This poses a potential risk to the health and safety of children in care.

This requirement was not met as evidenced by: In or about June 2021, there had been at least two different occasions that children were left unsupervised while staying in the front office area waiting for additional teaching staff to arrive to work in the morning. The children were in the hallway around the corner of the front counter and in the staff room without any staff member physically present with them in the same area. That presented immediate risks to the children's health and safety.

  • PERSONAL RIGHTS

    Personal Rights. The licensee shall ensure that each child is accorded...accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement is not met as evidenced by: In or about June 2021, due toinsufficient teaching staff to maintain proper teacher-child ratio in the class, some children were made to stay in the front office, staff room and the director's office with office staff for extensive period of time. That was a violation of the children's personal rights as those areas are not licensed as part of the child care activity areas and not age-appropriate for the children.
