Licensing Program Analyst (LPA) Taadhimeka Zeigler arrived at the facility on a case management visit to follow-up on an Unusual Incident Report (UIR) submitted to Community Care Licensing (CCL) by the facility on 11/16/2021. LPA spoke with the Acting Director, Brenda Sandoval and advised of the reason for today's visit. At the time of visit, LPA interviewed staff and children, and obtained facility records. LPA will continue the investigation into the reported incident. A final determination has not been made regarding any violations surrounding this incident. Exit interview conducted and a copy of this report and notice of site was left at the facility. A copy of this report must be made available to the public for 3 years.
