Inspector’s narrative

What the inspector wrote

On another occasion, a parent had instructed a certain diaper rash cream be used on their child due to them having sensitive skin; however, instead, a scented baby powder was applied causing the child to be in severe pain and unable to walk. It was learnt that due to facility staff not being able to find a child’s parent-provided diapering cream, another child’s diapering product was used, resulting in skin irritation to that child. During interviews, it was also discovered that due to infrequent diaper changes and/or a child not being cleaned properly, contributed to at least two children’s diaper rashes. After conducting interviews with pertinent parties, the allegation that facility staff failed to meet day care children’s diapering needs was confirmed to be true. Based on LPA interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC9099D. An exit interview was conducted with the Director, Christina Lambaren, Appeal Rights were discussed and issued, a copy of this report was provided, and a Notice of Site visit was issued. Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224 form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224). A copy of this report must be made available for the next three years.

Additionally, due to infrequent diaper changes and/or a child not being cleaned properly, contributed to at least two children’s diaper rashes, which poses an immediate Health, Safety, or Personal Rights risk to children in care.
