Inspector’s narrative

What the inspector wrote

On 06/05/2025 at 2:15 PM, Licensing Program Analyst (LPA) Raymond Moorehead arrived at the facility to conduct a case management inspection. LPA met with Director Tracy Bierman and Assistant Director Ann-Marie Schoben. A case management inspection was conducted in response to the receipt of an Unusual Incident Report (UIR) from the facility. The UIR was received by the licensing agency on 05/15/2025. The UIR documented an incident where a teacher at the facility alleged that another teacher at the facility handled a child in a rough manner. This was a self reported incident, and the facility's District Leader conducted an internal investigation regarding the allegation. It was also stated that the internal investigation lead to no substantial findings, but offered trainings for the alleged teacher. LPA investigated the incident, and pertinent interviews were conducted. The following was disclosed. A staff interview stated that a child was seen to have ran outside of the classroom. Additionally, it was alleged that the subject teacher from the classroom grabbed the subject child aggressively and pushed them back into the classroom. The subject teacher denied the allegation of handling the subject child in a rough manner. Further, the subject teacher stated that the subject child did run out of the classroom, and that the subject teacher hurried to grab the child for their safety. The subject teacher stated that this action was not out of malice and for the safety of the child in care. Based on the information obtained, there appeared to be no violations of Title 22 Regulations pertaining to the reported incident at this time. LPA conducted an exit interview with Director Tracy Bierman and Assistant Director Ann-Marie Schoben, and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days. No deficiencies were cited during today's inspection.
